Hemoglobin (Hb) is a vital metalloprotein found in red blood cells, primarily responsible for oxygen transport in the blood. Its proper functioning is crucial for maintaining health, while abnormalities can lead to various diseases.
In health, hemoglobin's primary role is to bind oxygen in the lungs and efficiently deliver it to tissues throughout the body, where it releases oxygen for cellular respiration. It also plays a secondary role in transporting carbon dioxide from tissues back to the lungs. A healthy adult typically has hemoglobin levels within a specific range (e.g., 13.5-17.5 grams/dL for men, 12.0-15.5 grams/dL for women), ensuring adequate oxygenation and overall physiological function. The molecule consists of four globin protein chains (two alpha and two beta in adult HbA) and four heme groups, each containing an iron atom that reversibly binds oxygen.
In disease, deviations in hemoglobin structure, quantity, or function can lead to significant health problems:
- Anemia: This is the most common condition associated with low hemoglobin levels, leading to reduced oxygen-carrying capacity. Causes include iron deficiency (most common), vitamin B12 or folate deficiency, chronic diseases, blood loss, and bone marrow disorders. Symptoms include fatigue, pallor, shortness of breath, and weakness.
- Hemoglobinopathies: These are genetic disorders affecting the globin chains. The most well-known are:
- Sickle Cell Anemia: Caused by a single amino acid substitution in the beta-globin chain, leading to abnormal, rigid, sickle-shaped red blood cells that can block blood flow, causing pain, organ damage, and severe anemia.
- Thalassemia: Characterized by reduced or absent synthesis of one or more globin chains (alpha or beta), resulting in ineffective red blood cell production, severe anemia, and often requiring lifelong blood transfusions.
- Polycythemia: Less common, this condition involves abnormally high hemoglobin levels, often due to an increased number of red blood cells. It can be caused by living at high altitudes, chronic lung disease, or certain bone marrow disorders, leading to thicker blood and increased risk of clots.
